Hello. Sounds like a good update. Keep up the good work. I'll be interested in the B12 level, too. Our Kolby did not start to gain weight until the B12 levels were up. So that might be the missing piece for your pup. Let us know.

Thanks for everyone's advice to date:
We finally have the B12 results = 68ng/L (pathology range of healthy dogs 252-908)and I am aware from this website that we should be aiming for 600+. Weekly injections for 4-6 weeks.
Tonight we are about to commence Tylan powder twice per day.
Weight - up 100 grams but that is over 3 &1/2 weeks
Food - 130 grams home made food 4 times per day (up to 1 1/2 scoops of Creon ) slowly introducing biscuits (4 tiny biscuits at a meal)
Poo - 95% of the time it is looking great (yellow/brown and the right firmness) the other 5% is usually something strange but no mucous or blood.
Happy, alert and bouncing around as though nothing is wrong.....despite still looking very very lean

Glad things are going well. WOW - that B12 is SUPER low! Once you complete the weekly shots please do not stop supplementing B12 - you can either continue with shots or use Chemeyes B12 pills or WonderLabs Pet Factor B12 - they both contain intrinsic factor which helps with B12 absorption. You have to find the right maintenance dose of B12 for your dog. A lot of us switch to the pills after doing the shots. Glad you are starting the Tylan as the yellow in the poop is likely indicating SID (small intestinal dysbiosis) is still at play.

Thanks everyone.
This weeks update - weight static
Tolerating the Tylan (and the small amount of cream cheese required to hide the taste)
Creon at 1 3/4 scoops per meal and working toward 2 scoops and then transition across to the capsules ($72 for 3 weeks verses $31).
Home made food continues and slowly introducing low fat/fibre biscuits (up to 8 tiny biscuits per meal)
Poo - 1 to 3 times per day, formed, yellow / brown and still has a slightly sweet smell rather the usual odour. I think that there is still the odd episode of consuming his own poo when we are not home.
Vitamin B - I am going to ask the vet if after Tuesdays injection I can start giving the weekly shot myself as it is only sub-cut and it will save a trip down the road. With his levels so low I am thinking that injections are likely to continue for a good 10-12 weeks before we can commence oral tablets (thanks for the advice on brands)

I feel like I am treating a "failure to thrive" human baby (without the night wake ups)

Thank you for the update.

Usually we do not advise adding any extra treats in to the diet until the dog is stable. What biscuits are you feeding?

What goes into your homemade food? Maybe an ingredient does not agree with him?

So you are saying it is cheaper to buy Creon in capsules than as loose granules?

Quick update-
Weight up 400 grams in 16 days!!! Only 1 kg to go...I imagine that is a result of increasing the Creon dosage per meal (to the equivalent of one 10,000 capsule), B12 injections and the Tylon.

Poo: still yellow/brown
Food: 4 homemade meals per day - chicken, pumpkin and broccoli. 6 small biscuits (kibble - total 10 grams). Total volume 130 grams per meal
No plans to drop the frequency as 4 meals is easier for us as a family to manage than 3 would be.

Enjoying reading the articles and others experience of this disease.

Well done with the weight are you still dosing the tylan twice daily morning and evening ? and a what dosage.The yellow brown may indicate SID is still present or it could be the food may be try sweet pots instead of the pumpkin see if that has any benefit .

Tylan is 1/8 tsp twice per day with a plan to go to the 45 day and reasses ? Continue longer term. I was thinking to try sweet potato- will do a couple of ice cube trays worth and do a test run to see if it affects output in anyway.
Once we have this under control and stable we need to look at having his deformed leg amputated as he can no longer be on anti inflammatories.
