Anal Fistulas

Hi, we're long time EPI owners and everything has been going really well for 8+ years but our boy has recently developed perianal fistulas. I've read up on treatment options and we're going to try just tacrolimus cream to start with but I just wanted to check- do I apply the cream actually on the fistula or just the area around it? They seem so open that I'm a bit unsure how I should apply the cream and I'm hoping someone here will be able to help
